If you are a new patient to the VHC Physician Group and have never been seen in any one of our practices, please click on the New Patient blue button below, fill in your required patient information in the light blue highlighted fields, print the completed forms and bring them to your first appointment.
Please remember to sign your name in the required signature fields on the respective forms. You may complete the forms online, print them and then bring those forms to your appointment as well.
Current VHC Physician Group Patients

You may complete the forms online, print them and then bring those forms to your appointment as well.
